Try the new NHS App
If you are a patient at our practice you can now use the new NHS App, a simple and secure way to access a range of NHS services on your smartphone or tablet.
You can use the NHS App to check your symptoms and get instant advice, book appointments, order repeat prescriptions, view your GP medical record and more.
If you already use our online services you can continue to use it. You can use the NHS App as well.
Patient Access is a website that displays information from your clinical record and allows you to:
- view and book available appointments
- view your medication and request further prescriptions
- view your medical record
- update your contact details
The website is available 24 hours a day and can be accessed from any computer, tablet or smartphone with internet connectivity. If you need further support there is an online support centre available to help you.
How to register for Patient Access
There are three ways to start the registration:
For all three options, you will need to visit the practice with proof of identity in order to get full access to your records. We will need to see three original forms of ID:
- One photo ID – such as a current passport, photocard driving licence, student ID, etc.
- Two documents e.g proof of your home address such as a utility bill or bank statement dated within the last three months (or two forms of photo ID and one document such as a utility bill).
- Read our Patient Access information leaflet
- Print and complete a Patient Access application form
- Visit the practice with your ID and completed form.
- You will be issued with a registration letter containing a unique Patient Access Linkage Key and Account ID,
- Visit https://app.patientaccess.com/registration to set up your full Patient Access account
Enter the practice postcode: YO21 1SD, select Search.
Select Whitby Group Practice, select Continue.
Below the question ‘Have you received a registration letter from your practice?’, select Yes.
Enter the Linkage key and Account ID from your registration letter, select Confirm.
Enter your personal details (must match the details held by your practice), select Continue.
Enter your email address (must not be a shared email), mobile number (optional), and a password of your choice (at least 8 characters, containing at least one upper-case letter, one lower-case letter and one number).
Select the box to accept the terms and conditions.
Select Register and create account.
You can register online for basic access, which will allow you to book a single appointment.
Visit https://app.patientaccess.com/registration
Enter the practice postcode: YO21 1SD, select Search.
Select Whitby Group Practice, select Continue.
Below the question ‘Have you received a registration letter from your practice?’, select No.
Read the information messages on-screen, then select Continue.
Enter your personal details (must match the details held by your practice), select Continue.
Enter your email address (must not be a shared email), mobile number (optional), and a password of your choice (at least 8 characters, containing at least one upper-case letter, one lower-case letter and one number).
Select the box to accept the terms and conditions.
Select Register and create account.
Request a Patient Access account below, and we will send you the forms. Remember you will need to visit with proof of identity in order to get full access.
