What is a community link worker?
A community link worker (CLW) works with your GP practice and is part of the practice team. They will help you access services to improve your health and wellbeing.
Services might include community groups in your area, advice about money and benefits, coping as a carer, parenting, looking after your own health, benefits, health and housing.
The CLW can also put you in touch with services that can help you talk through your worries or problems such as isolation and loneliness or difficulty with relationships.
What will the community link worker do?
The CLW will meet you face to face either at the GP practice or at another suitable place. They will support you with problems and issues you are experiencing and talk about what really matters to you. They will work with you and make a plan on how to access local services and helpful information to suit your needs.

Speak to your GP or any member of the practice team who will ask the community link worker to contact you.
