Register as a patient

We welcome new patients who wish to register with us.

To register with our surgery you will need to complete a registration form and a health questionnaire. The forms are available to collect from our surgery during our opening hours, or alternatively you may wish to download, print and complete a copy from our website. The appropriate form/s can be downloaded below or from our documents page:

• Patient registration form – GMS1
• Registration form for patients over five years of age
• Welcome pack – over five years of age
• Registration form for patients under five years of age

New patient check

Once you have provided us with your completed registration form, health questionnaire and identification please make an appointment with our Nurse or Health Care Assistant for a ‘new patient check’. As part of this check up, we will take basic measurements such as blood pressure, height and weight.
